REMEMBER, Please Arrive 10 Minutes Early

You will need to complete one of our confidential Medical History Forms.  This can take some time so please arrive ready to do this.  You are welcome to take a form home and complete it before your appointment if you wish.  If you pay NHS fees we will also have to process your payment before you enter the surgery for your appointment.  We therefore request that you arrive a minimum of 10 minutes early to ensure everything is sorted before your appointment begins.  This of course will help the practice run to time.



Some NHS patients are entitled to either full or partial exemption from charges.  If you think you may qualify, please ask.  You will need to provide evidence if you wish to claim full or partial exemption.


When you attend for an NHS dental appointment, before you are seen by the dentist, you will be asked either to provide evidence of exemption from NHS dental charges or to pay the relevant fee.


Cash, debit cards and most credit cards are accepted.  Cheques and American Express are not accepted.

Making an Appointment

If you would like to make an appointment, you can do so by contacting the practice by telephone on 01409 255525 or by popping in and speaking to the receptionist on the first floor.


You can express a preference about which dentist you would like to see you and we do our best ensure your request is met. However, we hope you understand that this is not always possible. If you do have a preference of practitioner, please let us know when booking your appointment.

Joining our Practice

There are three stages to joining and becoming a member of our Practice

  1. Providing us with your details (name; address; date of birth, etc)
  2. Booking a New Patient Examination
  3. Attending for and completing your New Patient Examination with the Dentist

Both stages 1. and 2. can be done over the telephone.


Only when your New Patient Examination has been completed are you a member of our practice.

Please note patients who only attend for emergency or casual treatment including patients that are referred by the Devon Doctors Service are not members of our practice.
